I hereby authorize Executive Driver Service to charge my credit card for services as stated in their contract for services.

Cancellation Policy Disclaimer:
Cancellation of any vehicle type less than twenty-four (24) hours prior to scheduled pick-up in the continental United States, and less than forty-eight 48 hours if international, will result in a full charge equal to the base fare price of the trip plus the service fee(s). If you do not see your chauffeur, you must call 1 (888) 525-7265 to avoid a “, no Show” charge which is equal to the base fare price of the trip plus service fee(s). Executive Driver Service shall not be liable for passenger flights, trains or cruise ships that are missed, cancelled, diverted, delayed or any other incident which results in failure to comply with the Cancellation Policy. In situations arising from cancellations and re-bookings, Executive Driver Service will do its best to fulfill clients’ needs without detriment to other reserved clients. Executive Driver Service terms and conditions may be changed at any time, without notice thereof, by updating this information. SPECIALTY VEHICLES: Reserved Stretch Limousines, Stretch SUV, Motor Coaches & Mini Coaches require 72 hours advance notice prior to the scheduled pickup time (based on the local time zone of the pickup location) to avoid a late cancellation or no-show penalty. Cancellation penalty within 72 hours of reserved pick-up time will be a charge of 50% of the total charge. Cancellation less than 72 hours will be a charge of 100% of the total charge. WAIT TIME CHARGES: There is a 15-minute grace period from the time of the requested pick up. Thereafter, a wait time charge (based on vehicle type) will be applied in increments of 30 minutes from the requested pick-up time until the vehicle is on the way to the designated destination. A copy of this Authorization is deemed as an original. Card Will Be Charged before Day of Service.
